Don't Forget to Sign Up for Orientation

Dear DeLaSalle Families,

It is an honor to write this letter to you as the new principal of DeLaSalle. I am privileged to be appointed to this position in a community that includes so many people committed to student growth and achievement. Even while many students are enjoying summer break, we continue to be busy at the school working hard to ensure a smooth beginning to the 2012-2013 school year.

Before the school year begins, you are required to attend one of the ten scheduled Islander Net Community (INC) orientation sessions. Thank you to those families who have already registered for one of the sessions. The orientation will provide an overview and update of the online Islander Net Community, put an iPad in your student’s hands, and review general expectations as we begin a new school year.

We understand that summertime can be a busy time for you with vacation and other activities. That is why we are offering 11 convenient orientation sessions. If you haven’t yet registered for a session, you can do so
here. Please know that registration will close on Thursday evening for the following week's orientation sessions in order to prepare each student's iPad. In other words, registration will close on Thursday July 26 for the orientation sessions on July 31 and August 1. iPads will only be issued to those families who have registered for an orientation because of the need to setup the device in advance.

Also at the INC Orientation session, we will ensure that you have completed all required documents and forms for the 2012-13 school year. Now that we have received updated versions of the documents, we have included a checklist and copies of these required forms with this correspondence. While you have registered to attend DeLaSalle and chosen classes, a student is not fully enrolled until we have these completed forms on file.

We ask that you complete and return the forms to us no later than July 27. Having all of the forms on file with us ahead of your scheduled INC Orientation will lead to a shorter orientation for you. We cannot issue an iPad to a student until all of the forms are completed.

If you cannot access a printer or are unable to retrieve these forms electronically, please let us know so we can provide printed forms to you. We will also have printed forms available at orientation for families to complete them that evening.

These forms are important because they provide up-to-date information about you and your student. The forms are also vital to our ability to file for government funds. During the 2011-2012 school year, DeLaSalle received about $220,000 in federal Title funds, which were used for counseling services, tutoring support, technology training for faculty, and other professional development activities. We also receive approximately $19,000 in E-rate funds annually to upgrade our telecommunication and information technology and communication services.

Checklist of Forms Needed for School Enrollment

The following is a checklist of forms that must be completed and returned to DeLaSalle no later than July 27. Having all of the forms on file with us ahead of your scheduled Islander Net Community (INC) orientation will lead to a shorter orientation for you.

You may have already returned the following two documents, but all families are required to have a “current” copy of each of these forms on file with us in order to be fully enrolled for the 2012-13 school year. If you do not have your form on file with us or are not sure whether the form is current, please contact the appropriate staff person below.

Tuition contract This document covers your family’s specific financial arrangements with DeLaSalle for the 2012-13 school year, including choice of payment plan and applicable fees, academic scholarships, and need-based grants for each student.  All families pay tuition to attend DeLaSalle, and all families are required to have a signed Tuition contract on file in the Business Office in order to be considered enrolled.

Physical form The Minnesota State High School League requires every student athlete to have a new physical exam every three calendar years (to the exact date of the last physical).

These following forms are just now available for the 2012-13 school year and required of all families on an annual basis. (If you are a returning family and have filled out these forms in past school years, you must still complete a new form for the 2012-2013 school year.)

Household Income Report The State of Minnesota and the U.S. Government require DeLaSalle to collect this information from you so that we may receive funding from both state and federal programs and services.  
Annual Health Information The DeLaSalle school nurse keeps up-to-date medical information about your child on file, including any allergies or vision or hearing problems he/she may have or medications he/she may take  
Student-Parent Handbook

Handbook forms (please return)
All families are required to to review the 2012-13 Student-Parent Handbook and be knowledgeable about school policies and procedures.  Related to the Technology Use policies of the handbook, there are four forms that you must sign and return to the school before your student can receive his/her iPad.  The forms include the Tablet Device Protection Plan, Student Use Pledge and Technology Agreement of Terms, and Handbook Acknowledgment  
MSHSL Eligibility Statement and Sports Health Questionnnaire The Minnesota State High School League (MSHSL) requires these forms of any student before they participate in any activity during the 2012-13 school year.  Please complete, sign, and return the last two pages of the document, which will be kept on file in the Student Activities Office.
