Athletic Physicals

The Minnesota State High School League requires every student athlete to have a new physical every three years. Doctor-completed physical forms should be mailed to the Activities Office at DeLaSalle as soon as possible.

Families are encouraged to visit their family doctor when a physical for athletic eligibility is needed. Please click the link for more information on free or low cost physicals at TRIA Orthopaedic Center, Fairview Clinics and Target clinics.

Physical forms must be turned into the activities office before the first date of practice. A blank physical exam form may be printed from the link or the clinic may have copies at their offices; check before going. All students who are interested in participating in an activity must complete the annual eligibility form. A blank eligibility brochure and form may be printed here.

Admission Fees

DLS students as well as adults and family members are reminded to always be prepared to pay an admission fee when attending activity events at other schools.

Admission prices for regular season home football, volleyball and basketball games is $6 for adults, $4 for non-DLS students(K-12), and $4 for seniors. Home regular season games and events are free for DLS students with a current DeLaSalle student ID card.

$50 All Athletic Event Pass

If you are a regular attendee at our sporting events, buying an all athletic event pass may be right for you! For $50 this pass will get one adult into our home regular season football, volleyball, basketball and wrestling events. You can purchase the pass at any of these home events at the admission table.
